Serum cystatin C-based equation compared to creatinine-based equations for estimation of GFR in patients with chronic kidney disease

Estimation of GFR is essential for the evaluation of patients with CKD. The Cockcroft-Gault and MDRD formulas are serum creatinine-based equations, and the most widely used tests for renal function. The present study compares own serum cystatin C-based equation and serum creatinine-based equations for 592 patients with CKD. Our results indicate that serum cystatin C-based equation is a reliable marker of GFR with a very high diagnostic accuracy and ability to predict patients with CKD and GFR under 60/ml/min/1.73 m2.

Kidney function estimating equations in patients with chronic kidney disease

764 adult patients with CKD were enrolled. Their GFR was estimated using five equations based on creatinine, Cockcroft-Gault, MDRD and the Chronic Kidney Disease Epidemiology Collaboration equation and two cystatin C-based equations [simple cystatin C formula (GFR=100/cystatin C) and our own cystatin C formula (GFR=90.63 x cystatin C -1.192)]. The GFR was measured using CrEDTA clearance. Our results indicate that both serum cystatin C-based equations are reliable markers of GFR in patients with CKD reliable markers of GFR comparable with creatinine-based formulas.

Serum cystatin C-based formulas for prediction of glomerular filtration rate in patients with chronic kidney disease

592 adult patients with chronic kidney disease were enrolled. Their GFR was estimated using five equations based on serum cystatin C ((Larsson, Hoek, Grubb, simple cystatin C formula (GFR=100/cystatin C) and our own cystatin C formula (GFR=90.63 x cystatin C -1.192)). The GFR was measured using CrEDTA clearance. Our results indicate that all serum cystatin C-based equations, excluding the Larsson formula, are reliable markers of GFR in patients with CKD and for daily clinical practice the simplest formula (100/cystatin C) could be appropriate enough for GFR estimation.

Serum cystatin C as an endogenous marker of renal function in patients with chronic kidney disease

Ocena glomerulne filtracije (GF) je pomembna za opredelitev ledvične funkcije bolnikov s kronično ledvično boleznijo (KLB). V pričujoči raziskavi so avtorji pri 252 bolnikih s KLB (GF < 90 mL/min/1.73 m2) primerjali ocene GF s pomočjo serumskega kreatinina, kreatininskega očistka po Cockcroft-Gaultovi formuli in serumskega cistatina C. Analiza korelacij in diagnostičnih zanesljivosti za različne označevalce GF je pri opazovani populaciji bolnikov pokazala superiornost serumskega cistatina C glede na serumski kreatinin.

Simple cystatin C formula compared to sophisticated CKD-EPI formulas for estimation of glomerular filtration rate in the elderly

317 adult Caucasians patients aged over 65 years were enrolled. We compare MDRD formula, CKD-EPI creatinine formula, CKD-EPI creatinine & cystatin formula and simple cystatin C formula (100/serum cystatin C) against 51CrEDTA clearance in the elderly. Our results indicate that simple cystatin C formula is reliable marker of GFR with high diagnostic accuracy and ability to predict GFR below and above 60 or 45 ml/min/1.73m2 in the elderly and is comparable to sophisticated creatinine formulas.
