Abstract
Chronic diseases and co-morbidities are a burden. Currently, patient and citizen information is available but fragmented. The EU-funded IMPROVE project will utilise patient-generated health data (PGHD) collected through m-health and e-health technologies. These data provide valuable insights into the real-life behaviour and challenges faced by patients of all ages suffering from complex chronic diseases and co-morbidities. The project will use this information to complement and enhance existing approaches to patient-centred outcome measures. The IMPROVE consortium will build a platform enabling the smart use of patient-generated evidence and input. The project will focus on 10 use cases in at least five different disease areas, such as ophthalmology, oncology, cardiovascular disease, chronic inflammation, and neurology.
